New Client Forms
I ask all new clients to complete a "client health form." In addition to basic information regarding general health, it asks questions regarding your preferences during a massage. It also asks for address and birthday information because I like to send birthday cards to current clients. It asks for e-mail and phone information so I can easily contact you in case we need to communicate regarding future appointments (confirmations, cancellations, re-schedules).
In addition to the client health form, I ask all pregnant clients to complete a "pregnancy health form."
I allow extra time during your first appointment for you to complete these forms in my office. I have provided copies here if you would like to print them out, complete them ahead of time, and bring them with you to your first appointment.
